Eaton Cooper Power Systems ELF Fuse Recall

Eaton Cooper Power Systems has issued a recall on their ELF Fuses manufactured from February 1, 2021–June 14, 2021. The recall is being issued because during one of Eaton’s routine internal tests, a nonconformance in the fuse end caps was identified that may prevent affected fuses from interrupting properly, requiring an upstream overcurrent protection to contain a fault.

Only fuses with a date code between February 1, 2021, and June 14, 2021, are impacted. Eaton Cooper Power Systems has sent Border States a list of purchase orders impacted by this recall. We have quarantined any impacted Elf Fuses in our stock and are working to have them returned and replaced. Border States Account Managers have already reached out to impacted customers. Eaton will be issuing replacements for all impacted fuses.

If you have any questions or concerns, contact either your local Border States Account Manager or your local Eaton Cooper Power Territory Manager.

– Description –
PVLV – Incorrect operation when controller transitions from Program to Run. Output to device is incorrectly generated when the controller is transitioned from Program to Run status. Expected behavior is that the PVLV function should not produce any output when the valve is at rest in the selected position unless there is an operator or program command to do so. This anomaly can specifically cause equipment damage when applying the PVLV instruction to motorized valves.
– Temporary Workarounds –
The workaround is to apply external logic (as shown below) to disable any errant signal from the PVLV instruction being propagated to the field.

Notice of Potential Product Concern
Rockwell Automation issues a Product Notice when it identifies a product(s) anomaly that may cause commercial or customer satisfaction concerns.
Basic Peer-to-Peer Communication False Detection of Link Failure with Potential for Shutdown of Trusted Controllers
Reference: 2021-02-004 Date: February 2021
This Product Notice informs you of a potential anomaly that exists with firmware for the Trusted® T8111 and Trusted 8110 processor modules. Basic peer-to-peer timeout values can expire within one application cycle and recover on the following application cycle.
This shutdown can be initiated when the input refresh status variables are set FALSE, this can occur in an individual Slave controller or in a Master controller. The peer-to-peer status signals immediately recover, but not before trip signals are initiated as if the peer-to-peer connection to the systems sourcing those trips had been lost.
Trusted T811X
PRODUCT NOTICE
expanding human possibility™
– Product Identification –
The affected product is firmware versions up through 4.000 for Trusted processors, catalog numbers:
• Trusted T8111C
• Trusted T8110*
Product and firmware version information may also be obtained electronically over a network connection. Refer to Rockwell Automation Knowledgebase Article ID 61300, Analysis Tool for Trusted Systems, for additional information.
Trusted T811X* Product Label
– Description –
An anomaly exists where the basic peer-to-peer timeout values can expire within one application cycle and recover on the following application cycle. This however can be long enough for link failures and data timeouts to be reported. If a multi-SIS controller architecture is configured to fail safe on loss of communications AND has a single SIS controller managing the entire basic peer-to-peer network, then this anomaly can result in shutdowns being initiated.
– Temporary Workarounds –
• For Simplex link applications – This workaround assumes that the “input refresh status” variable (see example below) is used in the application to initiate a trip, in accordance with the guidance provided in Table 21 of ICSTT-RM454). This variable (on each sxp2p_i board) is ‘locked’ and set TRUE (i.e., force this status variable healthy), then this will prevent the application generating a shutdown condition should this reported anomaly occur.
Trusted Catalog Number Location
PRODUCT NOTICE
expanding human possibility™
• For Redundant link applications – This workaround assumes that the “input refresh status” variable is used in the application to initiate a trip (logically AND ’ed with its parallel input refresh status variable for redundant link configurations), in accordance with the guidance provided in Table 21 of ICSTT-RM454). These variables (on each pair of sxp2p_i boards) are ‘locked’ and set TRUE (i.e., force these status variable healthy), then this will prevent the application generating a shutdown condition should this reported anomaly occur.

– Correction –
Correction requires updating product firmware to a later version. Rockwell Automation is working on a corrected version of firmware and will release it to the Rockwell Automation Product Compatibility and Download Center (PCDC) in the future. If you would like to receive a notice when this version is released, a link is provided at the end of the Knowledgebase article for this Product Notice.
Important: Correction requires a customer installed firmware update that will be available from the PCDC site. Controller hardware may not be returned for update for this notification. Upon installation of the corrected firmware, the workaround noted above must be removed.
– Requested Customer Action –
Rockwell Automation requests you take the following actions:
• Check if you have a product affected by this Product Notice. Refer to the Product Identification and Description sections of this document for product identification assistance.
• Until corrected firmware is available and can be downloaded and installed, follow the recommendations in the Temporary Workaround section of this notification.
• If you need additional assistance, please contact Rockwell Automation Technical Support. See Appendix A for local telephone numbers. Customers without TechConnect℠ support contracts should reference this Product Notice when calling.

– Description –
The SIL3/PLe configuration of GuardLogix 5580 controller and 1756-L8SP safety partner pair may experience a Major Non-Recoverable Safety Fault (T:14 C01, 03, 07 or 09) when online editing is interrupted.
An anomaly exists in the GuardLogix 5580 controller firmware (1756-L8xES and 1756-L8SP) where an online edit transaction may be mishandled between the safety controller (1756-L8xES) and safety partner (1756-L8SP). An interruption during an online edit may result in a Major Non-Recoverable Fault (MNRF) of the 1756-L8SP safety partner on the next online edit or on power cycling of the chassis containing the GuardLogix 5580 system.
The anomaly exists because the online edit transaction is removed from the 1756-L8xES controller but not the 1756-L8SP safety partner. When the next online edit or chassis power cycle is performed active transactions are checked for and the condition results in the 1756-L8SP experiencing a Major Non-Recoverable Fault (MNRF).
– Temporary Workarounds –
Customers should avoid interruptions to Studio 5000 Logix Designer® while performing online editing with GuardLogix 5580 controller configured as SIL3/PLe when using specified V31 and V32 firmware.
– Correction –
Correction requires updating product firmware for both the 1756-L8xES controller and 1756-L8SP safety partner. Rockwell Automation is working on corrected versions of firmware and will release them to the Rockwell Automation Product Compatibility and Download Center (PCDC) when available. If you would like to receive a notice when this version is released, a link is provided at the end of the Knowledgebase article for this Product Notice.
Important: The correction requires a customer installed firmware update that will be available from the PCDC site. GuardLogix 5580 Controller and safety partner hardware is not affected and may not be returned on this notification.
– Requested Customer Action –
Rockwell Automation requests you take the following actions:

ControlLogix 5580 Redundancy Unexpected Power-Up Handler Fault (Type 1 Code 1) with V33.011
Reference: 2021-02-002
Date: February 2021
This Product Notice informs you of a potential anomaly that exists with the ControlLogix® 5580 controllers with V33.011 Firmware. When using a power-up handler program in a ControlLogix 5580 redundancy system, you can experience a Major Fault (T01:C01 – Power-up Fault) on the new primary controller after switchover. To avoid this condition, remove the power-up handler and use the S:FS bit to execute the logic that was contained within the power-up handler.
This is a loss of control and redundancy.

– Description –
If a customer is using a Power-up Handler with a ControlLogix 5580 controller with V33.011 redundancy system and:
• The system is fully Qualified/Synched.
• The controllers are in run mode.
• Both racks are power cycled.
• The racks power up and fully Qualify/Synch.
• And a switchover is caused.
The “new” primary will have a major recoverable fault (Type 1 Code 1). This is a loss of control and redundancy.
The Power-up handler is only executed when the controller powers-up in Run Mode (remote or hard run mode). The Power-up
handler is typically used to handle conditions when the controller was powered down in run mode.
Note: By default, a Logix Controller application does not have a Power-Up Handler defined.

Rockwell Automation Library of Process Objects, P_PIDE Version 4.10.02 Critical Anomalies
Reference: 2021-02-001
Date: February 2021
Critical anomalies were identified with the P_PIDE version 4.10.02 of the Rockwell Automation Library of Process Objects. The anomalies relate to malfunctioning shedding actions when the P_PIDE is in manual mode and lack of bumpless transfer when transitioning from Automatic to Manual mode.
– Product Identification –
The affected products are the library objects associated with:
• Rockwell Automation Library of Process Objects (PlantPAx Process Library) – version 4.10.02
• RA Studio 5000 Application Code Manager Process Library Update – version 4.10.02
• Studio Architect Library Update – version 4.10.02
These products contain the affected library object – P_PIDE (version 4.10.02). The version number of the P_PIDE can be identified in Logix by navigating to the Add-On Instructions tab in the Controller Organizer and selecting the P_PIDE. The information in the Revision row indicates the version that is subject to this notice.
PRODUCT NOTICE

– Description –
Two critical anomalies were identified:
1. Shed Anomaly
The P_PIDE may not properly default its CV to the configured safe state value if the interlock conditions are not met while in the P_PIDE is in Manual mode.
“Safe state” actions can be configured within the faceplate to be None (do nothing), Hold (current state at the time of the interlock) or Use (a pre-specified value). “Interlocks” can be any number of specified conditions that are required for the P_PIDE to function normally in Auto mode. If the “chain” of interlocks is broken, the P_PIDE is expected to follow the behavior configured for its safe state.
The function works as expected when the P_PIDE is in Auto or Cascade mode but will not when the P_PIDE is in Manual mode, leaving the potential for unexpected machine/equipment behavior.
In addition, the operator can override the “safe state” during an interlock shed condition by simply placing the P_PIDE into manual mode and requesting a specific CV.

2. Auto to Manual mode anomaly
The transition of the P_PIDE from Auto to Manual mode is not bumpless as the CV is incorrectly driven to the last manually entered value instead of starting from the current CV.

– Temporary Workarounds –
There are no temporary workarounds identified that can be performed with the 4.10.02 version of P_PIDE. Customers are recommended to use an earlier 4.10.xx version of the P_PIDE (from library 4.10.01 or earlier) until version 4.10.03 or later is available for download and installation.
– Correction –
Correction required updating to RA Library of Process Objects version 4.10.03 or later. Version 4.10.03 can be obtained from the Rockwell Automation Product Compatibility and Download Center (PCDC) at https://compatibility.rockwellautomation.com/Pages/MultiProductDownload.aspx?crumb=112. In the “Search” field enter “Process Library”. Select the “Process Library” result, then the desired version. Follow the instructions to download and install the corrected version 4.10.03 or later.

Reference: 2021-01-001
Date: January 2021
This Product Notice informs you of a potential anomaly that exists with the CENTERLINE® 2100 NEMA LVMCC 100msec duration rated ArcShield™ R Frame. If control circuit power is lost, the panel door cannot be opened, and the internal circuit breaker cannot be operated.
PRODUCT NOTICE
expanding human possibility™
– Product Identification –
The affected products are CENTERLINE 2100 LVMCC catalog number 2193 with options TRUG (3000A R-frame breaker) and 112B (100ms Duration rated ArcShield). The full list of affected products by serial number is attached as appendix B.
Product may be identified by the serial number listed on the product nameplate. The serial number is shown in the CATALOG NO. / SERIAL NO. field.
In the example shown to the left, the serial number is 6504850087/100. This serial number matches one listed in Appendix B and is subject to this notification.
– Description –
A potential anomaly exists with the CENTERLINE 2100 NEMA LVMCC Duration rated ArcShield R Frame. If power is lost to the control circuit, the panel door cannot be opened, and the internal circuit breaker cannot be operated.
The ArcShield R-frame 100msec duration rated LVMCC relies on push buttons to power a shunt trip and shunt close using a line powered control transformer to operate the panel door interlock.
If the control circuit loses power with the circuit breaker ON, the following occurs:
• The buttons on the door become inoperative.
• The circuit breaker cannot be turned off.
• The door interlock cannot be released, and the panel door cannot be opened.
The user is unable to access manual buttons on the front of the R-frame circuit breaker with the current ArcShield design. There is no bypass built into the door. This product is the main incoming for a LVMCC lineup. The circuit breaker will remain ON continuing to power the installation.
– Temporary Workarounds –
ATTENTION: Only persons skilled in the maintenance of industrial control and electrical equipment should attempt to perform the following actions. Failure to follow industry standard safety procedures, including Lock-Out / Tag-Out (LOTO) and use of appropriate levels of personal protective equipment (PPE), may result in harm to personnel or loss of equipment.
If this anomaly occurs and the retrofit kit has not been installed, the following workaround will allow access to the cabinet.
PRODUCT NOTICE
expanding human possibility™
ATTENTION – You must remove power to the cabinet prior to performing these actions. Removing the catch angle disables the locking mechanism and can allow user contact with live circuits.
1. Disconnect power to the unit and perform Lock-Out / Tag-Out (LOTO) procedures.
2. Locate the hardware holding the catch angle to the door. Refer to the image shown at the left.
3. Grind or chisel off the catch angle hardware. This will cause the catch angle to fall into the cabinet, allowing the door to be opened.
The catch angle should be reattached to the door using standard nut and bolt hardware. Make sure the catch angle is reattached prior to restoring power to the cabinet.
Important – After performing the workaround actions, you must make sure all materials, including the catch angle and associated fasteners, are located and removed from the cabinet. Failure to do so may result in a short circuit when power is restored to the cabinet.
– Correction –
The correction requires the installation of retrofit parts. The instructions for the field retrofit are included in appendix C.
Rockwell Automation will provide retrofit parts for a period of 18 months after publication of this Product Notice. Customers should contact Rockwell Automation Technical Support to request a retrofit parts. When calling Technical Support it is important to reference this Product Notice – PN 2021-01-001.

Issue: A reset button to de-energize the R-Frame circuit breaker is missing from the door.
Impact: If a fuse on the control transformer opens, the control station that energizes and de-energizes the circuit breaker will no longer function.
If the control station is no longer functional, the circuit breaker can no longer be de-energized. The door lock on the circuit breaker will not release with the circuit breaker still energized.
Resolution: Install a reset button in the polycarbonate window of the door. The reset button will be positioned over the OFF button of the circuit breaker. This reset button will allow the circuit breaker to be de-energized if a fuse opens on the transformer. The photo below illustrates the position of the reset button on the door.

Section with the reset button installed
The following instructions show how to install the reset button in the window.
Reset button installation steps:
1. Before working on the section to install the reset button, the control station should be used to de-energize the circuit breaker. Before performing any additional work on this MCC, remove all power from this MCC after de-energizing this circuit breaker. This allows the door to be opened and access to the window.
2. A hole for the reset button will need to be added to the window. The current design has the window being held in place by (4) pieces of hardware. The hardware should be removed so that the window can be removed for alteration.
3. Once removed, the window will need to have a 0.50” (12.7mm) hole added to the window. The hole will be located at 3.06” (77.7mm) from the left edge and 4.02” (102.1mm) from the bottom edge. The image below shows the location that the hole should be located on the window.
PRODUCT NOTICE

4. Once the hole is added to the window, it can be re-installed on the door. The double-sided tape and the hardware used to mount the window on the door should be re-installed. When installed properly the hole will be in the bottom left quadrant of the door. Please refer to the picture at the beginning of this section for proper orientation.
5. Once the window is back in place, the reset button can be installed. The reset assembly will arrive fully assembled. The picture below shows the reset assembly and identifies the parts. The parts are (1) button screw, (2) spring, (3) bushing, (4) lock ring and (5) reset extension.
PRODUCT NOTICE

6. To install the reset, place the bushing into the hole that was cut into the polycarbonate window until the bushing sits flush in the window. The lock ring will then be placed on the bushing from the inside side of the door until it sits flush against the polycarbonate window. If properly installed the reset bushing should be held in place.
7. Once the bushing is in place, the spring can be placed in the bushing.
8. The reset screw is then place in the bushing to hold the screw inside the bushing.
9. With the reset screw and spring being held in place, the reset extension is then screwed onto the reset screw. If properly installed, the front of the reset screw should be flush with the front of the bushing. The reset extension should be held securely onto the reset screw. Refer to the pictures below to see what the installed button should look like.
